AI is everywhere. Dentists are curious, and many have already experimented: pasting patient notes into ChatGPT, trying transcription apps, or using consumer AI tools to draft clinical documentation.

  • Adding “bitewing radiographs taken” or “periodontal charting completed” by assumption.
  • Inserting the wrong material, like “composite” instead of amalgam.
  • Writing “patient tolerated procedure well” without evidence.
  • Stating “recall scheduled in 6 months” even if the patient left unscheduled.

On the surface, these look professional. But they’re false records—and in an audit, lawsuit, or board review, they can destroy your credibility.
