Upper-left scaling and root planing (D4341) and full-mouth adult prophylaxis (D1110) were completed with ultrasonic and hand instrumentation. Laser bacterial decontamination and antimicrobial irrigation were performed in upper-left periodontal pockets. Locally delivered antimicrobial agent (D4381) was placed at two upper-left sites.
